The Birth Survey

The Birth Survey is an initiative being undertaken by the Coalition for Improving Maternity Services (CIMS). By filling out the online survey, you provide the organization with feedback regarding your health care experiences with doctors, midwives, hospitals and birth centers. The group uses this information for the Transparency in Maternity Care project which allows women, their families, practitioners and other consumers to access online information about the quality of maternity care in their area. In addition to the information provided by childbearing families the survey also lists other important statistics such as intervention rates at various hospitals.
